HOW TO ENTER

Entry deadline extended to Monday March 5

  • Choose which category/ies you wish to enter – you can increase your chances of success by entering more than one category, but remember to tailor your submission to the specific entry criteria.
  • 2011 C+D Award winners cannot enter the category they won in 2011 but are free to enter any other category.
  • C+D has created an online entry form below to make the process easier. It allows you to enter multiple categories, save and edit entries, as well as upload supporting material in four simple steps:

Register your details below.


Step 2

Select the category you wish to enter and complete the secure online form. You can store your entry online and edit it as often as you need before you submit it. You can also upload up to 5MB of supporting material with your entry.


Step 3

Submit the entry when you are finished with one click.


Step 4

Repeat Steps 2 and 3 to enter another category.

Terms & conditions of entry

Your submission must not exceed the word counts. Where appropriate, you can upload up to 5MB of supporting material (clearly labelled) such as testimonials, research, performance metrics, service protocols, press clippings etc. These can enhance your chances of winning, but are no substitute for a compelling entry submission. Supporting material does not count towards the word limits.


Clearly written entries with supporting facts will score highly with judges. Please include evidence in your answers, citing figures/ data/metrics wherever possible, for example: how many patients were eligible for the initiative, how many patients your initiative served, how many patients were referred to another healthcare service, how much customer awareness was raised, how much footfall/ turnover/related sales/ prescription volume increased by, what targets were met/exceeded.


Entries without appropriate supporting evidence such as applicable financial information will not be shortlisted, as such information forms an essential part of the judging process.
