Due to the popularity of this award, this year the category has been split into two to give both individuals and teams a chance to shout about their achievements.

Tell us about a time when you or your team went beyond the call of duty to help patients – from life-saving interventions to charity work and everything in between. The shortlisted stories in each category will be voted for by C+D readers.

Unlike other C+D Awards, this isn’t about assessing an individual’s career or an entire project. Instead, this is about one specific event that allowed you, your team or a colleague to shine.
