How to Enter
This category consists of nine previous winners, C+D readers will get the chance to vote for the winner in a live poll on the C+D website. The winner will be announced at the C+D Awards in July.

To celebrate the 10th anniversary of the C+D Awards, we have created a special category for 2017 – and are giving you the chance to pick the winner. Over the coming weeks, we’ll be looking back at each of the past recipients of the coveted Community Pharmacist of the Year trophy. Readers will then be given the chance to vote for their favourite entry, with the winner announced at the C+D Awards ceremony at Celtic Manor in Wales on July 12.

Why Enter?
This award combines the two previous award categories of Public Health Initiative and Clinical Service of the Year. With the nation facing a raft of ticking healthcare bombs, public health and clinical services are at the heart of the NHS and pharmacists are central to delivering them. Whether you provide, for example, public health services such as a substance misuse service, an anticoagulation clinic or palliative care, or clinical services such as smoking cessation, diabetes screening or flu vaccinations, it goes without saying that your service has to be well-executed. But it also has to demonstrate that it meets a local health need – so tell us how you identified the need and how your service met it. And, most importantly, tell us how your service has made a difference to your patients – whether it’s cutting GP appointments or hospital admissions, increasing access to services or improving health outcomes.

Why Enter?
Every independent pharmacy has to balance the demands of delivering a modern, patient-centred health service while offering a customer experience that rivals the big high-street retailers. And the best independent pharmacies have turned this into an art form, with warm inviting premises, welcoming and knowledgeable staff and accessible health services. Winning this award is not about how much you’ve spent, it’s about what you’ve done with your investment. Whether you have, for example, undertaken a top-to-bottom revamp of your pharmacy, launched a successful private health service, used your IT systems to increase your efficiencies, or consistently hit every objective, tell us how you and your independent pharmacy business’s team have propelled it from the also-rans to the top of the podium.

Why Enter?
Making the transition from classroom to frontline practice is a big step for pre-registration graduates. But the best students make it look effortless, as they turn the theoretical knowledge gained at university into the practical application of patient care. The winner of this award won’t be your average pre-reg; they will have set themselves apart from their peer group with their drive, ingenuity, ideas and commitment to make a difference to their community. Tell us what you have done and why, how you did it and what you have learned from the experiences that will help you in your future career.
Who Can Enter?
Anyone who was on a pre-registration training placement with a community component during 2016. This can include both those who began their placement in 2015 and completed it in 2016, and those who began their placement in 2016 and are due to complete it in 2017. Either way, the majority of the work referred to in your entry must have taken place during 2016 and in the community component of your placement.

Why Enter?
Integrated care is the holy grail of modern health service provision – and the only way to achieve it is for professionals from different disciplines to work together. For community pharmacy, a key partner is general practice, not least because GPs now hold local commissioning purse strings through CCGs. So whether you’ve joined forces with your local surgery to identify patients at risk of diabetes or developed a forum for sharing information and ideas between practice and pharmacy teams, it’s time to show how GPs and pharmacists can be friends, not foes – to the benefit of their shared patients.

Any multidisciplinary group, including at least one GP and one member of a community pharmacy team, who have improved patient care by working together. And, as the answer to the question of how pharmacies can embrace the service agenda or survive funding pressures is often ‘skill mix’, your roles are increasingly in the spotlight themselves. Great businesses never stand still. They continue to innovate, evolve and raise the bar for competitors. And it’s not just about throwing cash at problems - it’s about the creative ideas that genuinely move your business forward. You might have, for example: improved your dispensing service through new workflow processes; unlocked the wealth of data in your PMR system to launch targeted support services; utilised robotic technology to free up your pharmacy team; or won a commissioning bid. Whatever your award-worthy business initiative, the judges will want to see how your business and, most importantly, your customers have benefited.

Any individual or team working for or on behalf of a community pharmacy business. Please bear in mind that while you can enter as many categories as you like, e ach entry can only be submitted for one category. If your business initiative primarily involves using the internet in some way, it may be best suited for the Online Initiative category. If your initiative is primarily focused on refitting our pharmacy, it may be best suited for the Pharmacy Design Award instead.
