Practice nurses and healthcare assistants in GP surgeries deal with the majority of chronic disease management doing bloods, bp, respiratory stuff etc..Only refering petients when there is a blip and the GP intervenes.

I do think what community pharmacists can contribute is minor ailments, medication review and getting an SLA for monitoring patients that GPs feel need extra support with medicines.
