It may be better than it was but it is still pretty confusing. Inevitably all employed community pharmacists will now be under immense pressure to deliver on NMS target as well as MUR targets where has our professionalism gone? Pharmacy is in a very sad place at the moment and I hope one day pharmacists will have their skills utilised more appropriately. The risk is we will never be taken seriously again, instead we will be seen as ruthless money grabbers trying to achieve unrealistic targets.

The new payment structure is certainly a lot more coherent but one consequence of this will be for employee pharmacists to be put under more pressure to meet these NMS targets.

The payment structure could be kept as simple as £25 per NMS but saying that I think these are GREAT NEWS: we will be paid for ALL NMS completed and even better, the payment is going to be backdated!

I think whilst this is not a simple as it could have been, it is still a step in the right direction.

When I was at the Pharmacy Show in October, I asked Sue Sharpe about NMS. On here I had criticised PSNC, but today I would like to say that they have done better.

I now hope that this will provide incentive for pharmacies and pharmacists to carry out NMSs. 2 of our pharmacies were able to get the implementation fee.

As for targets, are you all talking about "stick" approach from employers/contractors or the "carrot and stick" approach?

One thing that I am not happy with is when patients are being told to return to their GP in 2 weeks time (so why would patient want consultation with pharmacist). You will find this in rural areas more... and makes it difficult for pharmacists to get motivated to do NMSs.

This is very positive and shows how NHS Employers recognise that we need a realistic target to motivate contractors. It's tough being a contractor just now but I can tell you it's just as tough being an employee pharmacist - "I am ringing to see how many MURs / NMS you've done this week".

NMS is a fantastic service and now contractors get paid fairly for it. Can we say the same for employees?

I appreciate that the change is in the right direction, but am amazed that in over 3 months of discussion this "unfair mess" has merely transformed into a "fairer mess" . By all means have an additional element reflecting volume, but it is nonsensical to pay such differing amounts for essentially the same piece of work being done each time. The NMS fee should be fixed and reflect the time and expertise done into it. Any other factors like volume should be reflected elswhere.

I think this new 'improved' system awful. It is still grossly unfair.
To date, including March, I have achieved a maximum of only 40% of the target. Yes, I have missed some, the last being a 6 month old being put on Ventolin, not sure how I could have got consent etc.
At a review meeting with the local GP practice only 10 days ago, they asked about NMS. When I said that I was only achieving 40% of targets, they replied that they were a conservative prescribing practice (and proud of that fact). Lots of the NMS have been swops from Valsartan or Candersartan to Losartan, cost saving initiatives from the PCT. Otherwise the figure would have been much lower.
There were fewer 'real' new drug swops or changes.
So, I do the initiation, intervention and follow up but can only get £26, whereas others will get £28 for the same work.
This is definitely NOT fair.

ok, so, assuming i do between 7,501 and 8,500 items a month and i manage to complete 15 nms, will i be paid for the 15 or the 8 nms (notice that 16 is the target to reach the 40% target)? if i get paid for 8 nms then what is fairer about the new payment structure? forget the payment for each episode still all the extra work and time will be spent for free. wasn't this the main problem of this service? correct me if i'm wrong please. thanks

You will be paid for 15 nms's at target rate of 20% which is £25 per NMS.

And that's the problem which PSNC still seem unable to get the DH to see. Why not the same fee per NMS. You have to do the same amount of work per patient whether you get paid £20 or £28.

I bet PSNC fob us off with statements like "the DH would not sign off on a service where there was no differential payments based on volume." What next? The basic payment per item fee?
