Pharmacists 'vital' to improving post-discharge care, NHS England says

NHS England has urged pharmacists to help tackle problems in post-discharge care, which contributed to 10,000 patient safety incidents in one year.


The commissioning body told C+D on Friday (August 29) that pharmacists had a "vitally important role" in managing post-discharge medicines and helping to iron out the lack of communication between secondary and primary care settings.


The call came after NHS England issued a patient safety alert on the same day, which set out plans to improve the "complicated" care of post-discharge patients by gathering feedback and best practice examples from healthcare providers. Although the alert did not mention pharmacists specifically – instead naming GPs and community nurses – NHS England told C+D pharmacists were "central" to its work.


"The professions mentioned in the alert are intended as examples only and certainly not intended to exclude pharmacists, who should rightly be recognised as central to the work on medicines optimisation following discharge," the commissioning body said. 10,000 patient safety incidents related to discharge were recorded between October 2012 and September 2013, a third of which were down to problems in communication, it said.



NHS England urged pharmacists to get involved in the initial phase of its programme (see boxout) to improve post-discharge care, which calls on healthcare professionals to share best practice, answer a questionnaire on their experiences and to make someone in their organisation responsible for keeping up-to-date with the programme.


NHS England also expressed hopes that its upcoming webinar on medicines discharge, details of which are yet to be announced, would be "well attended" by pharmacists.


Initial stages

The initial stage of the programme is due to be completed by October 13. The next stage will involve NHS England providing a "range of resources and recommendations" to help organisations improve post-discharge care at a local level.

Last week, C+D reported that a pharmacy service for recently discharged patients in Wales offered a three-fold return on investment but suffered from a lack of awareness.



Improving post-discharge care: how you can get involved


1) Make someone in your organisation responsible for keeping up-to-date with the programme and email their details to [email protected]. They will be notified about upcoming webinars on post-discharge care.


2) Share the details of any work you are undertaking or have undertaken to improve post-discharge care on NHS England's online template.


3) Complete an anonymous online questionnaire to help identify national priorities for improving post-discharge care.


4) Share the alert with any voluntary organisations that you collaborate with in caring for patients discharged from secondary care.
