Health Initiative of the Year
Whether you provide public health services such as an anticoagulation clinic or palliative care, or clinical services such as smoking cessation, diabetes screening or flu vaccinations, it goes without saying that your service has to be well-executed.
Any individual or team that has delivered a public health or clinical service with a community pharmacy component. Please note that each entry must be specifically tailored to the relevant criteria and the same/similar entry cannot be submitted for two categories. For entry help and advice, register to receive our Hints & Tips.
If your entry is regarding a specific individual who has promoted the public health agenda in their area, then this may be more suitable for 'Public Health Champion of the Year'.
In no more than 300 words per question, please answer the following:
- How did you identify the need for your public health or clinical service, and how did it meet this need?
- What impact has your health initiative had on your patients?
- What impact have you had on your pharmacy/ies and/or your pharmacy team(s)?
- How has your health initiative improved your pharmacy’s/ies’ links with other healthcare professionals?
