Why am I getting cramp in my calves?

Ron Millard is a 78-year-old man with a 15-year history of type 2 diabetes, hypertension, hyperlipidaemia and high BMI (body mass index). He gave up smoking cigarettes 10 years ago, but has the occasional cigar now and again.

He takes the following medication, all of which have remained unchanged for the past four years:

  • Metformin 850mg bd
  • Gliclizide 80mg bd
  • Simvastatin 80mg on
  • Ramipril 10mg od
  • Amlodipine 10mg od
  • Humulin M3 insulin 20 units pm
  • Aspirin 75 mg od

Mr Millard comes into the pharmacy today asking for something for leg pain. He tells pharmacist Ramesh that, on several occasions, he has experienced a severe cramping pain in the back of his calves. Up until now it has only happened when he has been playing bowls, however, yesterday he was walking round the shops
